Degree Certificate (issued by the University).

Course Completion Certificate (issued by the Dental College).

Transfer Certificate (issued by the Dental College).

Mark Sheet- First year (to) Last year take a (A4 size) colour xerox in a separate page (issued by the University).

Candidate has to come with the Gazette Notification for the MDS Course with Specialty branch Recognition issued by Ministry of Health & Family Department, New Delhi. Notification copies submit at the time of Registration.

Two recent colour passport size photos- same photo uploaded in the online should be submitted at the time of Registration.
