DENTAL HYGIENIST / DENTAL MECHANIC CERTIFICATE (issued by the Dental College).

Course Completion Certificate (issued by the Dental College).

Transfer Certificate (issued by the Dental College / School).

Nationality (Indian) Proof, Community Certificate & Valid Indian Passport only accepted.

HSC Mark Sheet (issued by the School).

Two recent colour passport size photos- same photo uploaded in the online should be submitted at the time of Registration.

Aadhar Card is must. (If Aadhar is not available, optionally you can submit Voter id or Valid Indian Passport are only accepted). Any one of the proof with Front & Backside taken in a single page – Colour (A4 size) Xerox must be submitted.

If a candidate arrives after One year and above, and received the Dental Hygienist certificate and came for registration, they have to get an affidavit from Notary Public for the delay of registration.

Handwritten letter from Candidate’s stating the delay of registration, same as mentioned in the affidavit also.
