Take a (A4 size) colour printout of the application form in a single side (Don’t take front and back).
BDS Provisional Certificate - 2nd after finish CRRI (issued by the University).
Compulsory Rotatory Residential Internship (CRRI) Certificate - (issued by the Dental College).
Transfer Certificate - (issue by the Dental College / School).
Nationality (Indian) Proof - Community Certificate & Valid Indian Passport only accepted.
Two recent color passport size photos - Same photo uploaded in the online should be submitted at the time of Registration.
Aadhar Card - Aadhar Card is must. (If Aadhar is not available, optionally you can submit Voter id or Valid Indian Passport are only accepted). Any one of the proof with Front & Backside taken in a single page – Color (A4 size) Xerox must be submitted.
Candidate’s from Andaman & Nicobar islands - For registration have to submit the Andaman & Nicobar islands Id card issued by Government of India.
More Than One Year Delayed BDS Registration

If a candidate arrives after One year and above. He / She received after provisional certificate and came for registration. They have to get an affidavit from Notary Public for the delay of registration.
Handwritten letter from Candidate’s stating the delay of registration same as mentioned in the affidavit also.
Two recent color passport size photos - the same photo uploaded in the online should be submitted at the time of Registration.
