DH/ DM REGISTRATION
Before Registration Read Instructions carefully. Payment once made will not be refunded.
INSTRUCTIONS
- The applicant should come in person to the office.
- All particulars of the application must be filled by the applicant.
- The name and particulars entered must be exactly correspond with the name and particulars of the applicant entered at the University.
- Directly scan the original certificates in colour and upload in the online application
NEW REGISTRATION
- Documents to be submitted along with the DENTAL HYGIENIST / DENTAL MECHANIC Application Form
- All Original Certificates & Address proof take (A4 Size) Colour Xerox copy only. It’s Mandatory.
- Take a (A4 size) colour printout of the application form in a single side (Don’t take front and back).
- DENTAL HYGIENIST / DENTAL MECHANIC CERTIFICATE (issued by the Dental College).
- Course Completion Certificate (issued by the Dental College).
- Transfer Certificate (issue by the Dental College / School).
- Nationality (Indian) Proof, Community Certificate & Valid Indian Passport only accepted.
- HSC Mark Sheet (issued by the School).
- Two recent colour passport size photos- same photo uploaded in the online should be submitted at the time of Registration.
- Aadhar Card is must. (If Aadhar is not available, optionally you can submit Voter id or Valid Indian Passport are only accepted). Any one of the proof with Front & Backside taken in a single page – Colour (A4 size) Xerox must be submitted.
- Candidate’s from to submit the Id card issued by Government of India
MORE THAN ONE YEAR DELAYED DH/DM REGISTRATION
- Documents to be submitted along with the DH / DM Application Form
- All Original Certificates & Address proof take (A4 Size) Colour Xerox copy only. It’s Mandatory.
- Take a (A4 size) colour printout of the application form in a single side (Don’t take front and back).
- Dental Hygienist / Dental Mechanic Certificate (issued by the Dental College).
- Course Completion Certificate (issued by the Dental College).
- Transfer Certificate (issued by the Dental College / School)
- Nationality (Indian) Proof, Community Certificate, Valid Indian Passport only accepted.
- HSC Mark Sheet (issued by the School).
- If a candidate arrives after One year and above. He / She received after Dental Hygienist certificate and came for registration. They have to get an affidavit from Notary Public for the delay of registration.
- Hand written letter from Candidate’s stating the delay of registration same as mentioned in the affidavit also.
- Two recent colour passport size photos- same photo uploaded in the online should be submitted at the time of Registration.
